The Charlotte White Center (CWC) provides case management services to children with a diagnosis of intellectual or developmental disability, mental health disability, autism, pervasive developmental disorder, or in situations where there has been a mental health crisis. These services are intended to connect affected individuals with the skills and resources necessary for them to reduce or recover from any medical or behavioral symptoms that negatively impact the person’s quality of life.
Team Planning
Utilizing consumer strengths, including a focus on family, our highly qualified staff works to create a partnership between the child/adolescent and their family, identifying and prioritizing areas of concern. This newly formed team works to maximize its potential by determining the individual’s strengths and needs, and the presence or absence of skills and resources necessary for them to achieve their goals.
Our Case Managers then monitor the plan to ensure flexibility, so it remains relevant to the individual’s specific needs as progress occurs and changes develop.
Strengths Based
CWC seeks to make the best use of the child/adolescent’s natural support system by using the following strengths based approach:
Referral Process
We encourage referrals from individuals, family members, medical community, case managers, DHHS, service providers, schools and local hospitals.
Once referrals are received, a CWC staff member will contact you and schedule an appointment for initial intake. The intake procedure involves gathering all the data necessary to determine eligibility, such as previous evaluations and test results.
Areas of Service At present our service network extends to Piscataquis, Penobscot, Hancock, Franklin, Somerset, and Waldo Counties.
