• Room and Board Services:

  • Charlotte White Center Assisted Living meets the safety, fire safety, physical plant sanitation standards of the Regulations Governing the Licensing and Functioning of Assisted Living Facilities

  • Diet and nutrition that meet the dietary services, nutrition and health standards of the Regulations Governing the Licensing and Functioning of Assisted Living Facilities

 

Eligibility 

• 18 years old or older 

• Primary diagnosis of developmental disabilities

• Must be referred by the Department of Health and Human Services

 

Charlotte White Center agrees to cooperate with the attending duly authorized licensed practitioner and other Health and Community Service Providers of the resident’s choice and to carry out the medical orders and treatment plans as directed, except where such orders or plans are in violation of the Regulations Governing the Licensing and Functioning of Assisted Living Facilities.
