The following 24/7 rehabilitative care services are provided as needed by the individual's needs:

Assistance and supervision of activities of daily living: bathing, dressing, eating, toileting, ambulation, personal hygiene, grooming and household tasks

Maintenance of health and safety: Personal supervision or monitoring; observing and reporting concerns regarding behavior or health to medical providers or supervisory personnel; crisis prevention and self-management; assistance with administration of physician ordered medication

Communication and social skills; Conflict resolution

Accessing community resources, vocational educational and recreational opportunities

Help with transportation and arranging appointments with medical providers

Ready for discharge from an institution, or recently discharged, or in need of a supported living environment

Waiting List
Individuals on our waiting list are reported to DHHS on a monthly basis. Individuals who are currently in a hospital or are at risk of being hospitialized are the highest priority. The waiting list is updated at least every 30 days.
Staff Qualifications
CWC Behavioral Health Residential Employees have a Mental Health Rehabilitation Technician (MHRT) 1 Certificate, as required by DHHS Division of Licensing. This includes being certified in First Aid, Mandt CPR and Medication Administration.
