Charlotte White Center provides conflict-free Care Coordination Services for adults with acquired brain injuries.  This integrated service is designed to support individuals, in their homes or community, while they develop and work on goals focused on achieving the greatest degree of self-sufficiency and independence.  Individual choice is paramount to the delivery of this service, from choosing support services, to setting personal goals, and accessing desired services including Career Planning, Assistive Technology, and Employment Specialist Services.

 

Eligibility

An individual is eligible for this program if he/she meets the following criteria:

· 18 years old and legally reside in Maine

· Diagnosis of Acquired Brain Injury and other related diagnosis

· Meets MaineCare eligibility requirements

· Does not receive services under any other federally approved MaineCare home and community-based waiver program and not currently receiving any other services from Charlotte White Center

· Can have his/her health and welfare needs met in a community setting

· Referrals come directly from Office of Aging and Disability Services(OADS)

 

Services Provided

  • Assist individuals in gaining access to needed waiver and State Plan services, as well as medical, social, educational and other services, regardless of the funding source for the services to which access is sought

  • Assist individuals to access and coordinate natural supports, and  monitoring and assurance of the implementation of the Care Plan. This includes monitoring of the health, welfare and safety of the individual

  • This service requires face-to-face contact between the Care Coordinator and the individual, at a minimum of every thirty days
